For Pre-School (Nursery, Kinder and Prep)

a. PSA Birth certificate
b. 2 copies of colored ID pictures (2×2)
c. Medical Certificate
d. Original Copy of Progress Report Card (Form 138) for Kinder and Prep
e. Recommendation letter or Certificate of Good Moral Character from school they attended (Kinder and Prep)
f. For Special Children – Assessment form coming from their Occupational Therapist, Pediatrician or Neurologist.
